Tri-City Lacrosse requires US Lacrosse insurance for all players. Renew or enroll in the program here.

NOTE: Players from the Utica/Rome area should enter "Tri-City Lacrosse" for the US Lacrosse Chapter. Also, take note of your US Lacrosse ID. This number needs to be entered on the form below. Print out your completed insurance form and bring it to your try-out (new players) or first scrimmage or game (returning players).

You can't participate in a Tri-City Lacrosse event without this insurance.

You can pay by check to take advantage of the early signup special. Please ensure your insurance is up to date and that you've filled out and sent the form below. You must bring a check to the draft (new players) or your first scrimmage (returning players) or a fee of $115 will be required.
